Residency Documentation Requirements

Residency Verification Documentation

Proof of residency requirements are listed below which includes
documents from category I, II, and category III. In addition, NEW
students must also bring a birth certificate and Illinois Transfer Form
(if coming from another Illinois school). A hospital certificate is not
sufficient... We will make a copy and return your original document.

Category I
(One document showing proper name and address.)
Mortgage statement

Blue housing assessment form from
the Lake County's Assessor's Office

Current real estate tax bill

Closing statement for purchase
of residence

Current and updated signed lease. If the current lease cannot be provided, a month to month addendum form, completed by the landlord indicating
current occupancy is required.

If you do not own or rent property... the OWNERS of the property must complete a Residency Affidavit and Addendum form.
This form must be notarized.
Category II
(Two documents showing proper name and address.)
Gas, electric, water, cable/internet bill in the last 60 days
Home/Renters insurance
Car insurance or car registration
Bank Statement

Paystub

Tax Return

Voter Registration Card

Public Aid Card
Category III
Valid Driver's license

State ID or Valid Photo ID

Passport/Resident Card
