Referral Process for a Case Study
Any person wishing to refer a student suspected of being eligible
for special education services should make a written request to the
building principal. The classroom teacher or principal will then
contact the parent to complete the referral packet in a timely
manner. A meeting will be held for the referral completion. The
parent signature for permission for a case study evaluation is
obtained. The Family History and Student Development form is
completed by the parent. The parent is given a verbal explanation
and a copy of "Explanation of Procedural Safeguards Available to
Parents of Students with Disabilities." A copy of the Case Study
Evaluation Components is also explained and given to the parent.The completed district referral packet is reviewed and signed by the special education coordinator and is assigned to the Multidisciplinary Team.
In cases where the request is not in the best interest of the child, a copy of the Parent Guardian Notification of Decision not to Conduct a Case Study Evaluation is given to the parent along with a copy of the Explanation of Procedural Safeguards.
The building principal, teacher, and MDC team members receive written notification of the referral assignment.
